Screening Form Builder
In "Study Settings," select the “Screening Forms" tab from the menu on the left and choose “Add New Form”:
By default, your form will be called “Form 1.” You can change the name of the form by simply highlighting the title “Form 1” and typing your own title.
Internal Screening Form: Makes your form available within the potential participants tab so that users can do phone screenings or in-person screenings with the Ripple application open.
External Screening Form: Makes your form available on the web so that potential participants can access it via web-link and fill out the information you requested.
After giving your form a title, you can customize the form in several ways included hosting it internally or externally, and adding any number of questions, instructions, headings, separators, and pages. The form creation was designed to give you the flexibility to create the form with the look and feel you desire. Any time during the form creation you can click the “Open as a live form” button to preview what the form will look like during use.
Questions can be written in any way that you choose, but the answer must be linked to one of the participant requirement fields that appear in the drop-down list. The drop-down list is populated with general requirements (First Name, Last Name, Global ID, Family ID, birthday), contact requirements (Contact Name, Relation to Participant, Email, Phone, Address), group requirements (race, ethnicity, sex), and custom requirements (custom variables created in Ripple). If you plan to ask questions related to custom requirements those custom variables must be created within the study (or globally) prior to building the form.
When information is entered into your screening form, the data that is entered will be entered into the variable of the profile card to which the question is linked. For example, if age is entered, the age field in their profile card will be populated with the number they enter after the form is submitted.
By default, all questions are required to be answered on your form. You can change this by "unchecking" the box next to required on the form. Of note: First Name and Last Name are always required in the Ripple application. If you decide to include these questions in your form they cannot be unchecked.
You can create as many questions as you would like in the form.
Instructions are text blocks that are not tied to a question. They can be used as instructions:

You can create multi-page forms by selecting “Add Page”. Each page is then designed by adding Questions, Instructions, Headings, and Separators as described above.
Pages are navigated on the screening form by using the "Next" button:
